Introduction. The Republic of Dagestan is among the regions of the Russian Federation with a consistently high proportion of people over 90 years of age and ranks first in life expectancy (79.8 years). Despite the demographic significance of the longevity phenomenon, the existing procedures for providing primary health care do not take into account the specific characteristics of long-livers and the prevalence of comorbidities, including geriatric syndromes. This study requires adapting the regional geriatric care model to the needs of long-livers. Objective. To consider the possibilities of optimizing regional geriatric services for long-livers (90 years and older) in the Republic of Dagestan. Materials and Methods. This review article analyzes official statistics and reviews scientific publications on the problem of polymorbidity and geriatric syndromes in individuals 85 years and older. Results and discussion. It has been established that the prevalence of chronic pathologies in the 85-year-old and older age group reaches 82%. However, the current geriatric care system for centenarians fails to account for the high risk of polymorbidity, polypharmacy, falls, cognitive impairment, and other conditions in this patient group. The need to optimize the regional geriatric care model is substantiated: introducing screening for geriatric syndromes, implementing preventive programs, developing interagency cooperation, and telemedicine for mountainous areas where a significant proportion of centenarians reside.
