According to the forecasts of experts and researchers in many countries, the proportion of elderly patients suffering from cardiovascular pathology and, above all, myocardial infarction and arterial hypertension will continue to increase, which will be accompanied by the accumulation of geriatric syndromes that significantly worsen basic and instrumental functional activity, the study of which is not given sufficient attention in the nosologies under consideration. Purpose - to study was to optimize the prediction of the outcomes of myocardial infarction (MI) in patients with frailty on the background of arterial hypertension (AH). To assess the prognostic significance of geriatric syndromes and indicators of endothelial dysfunction studied in the period from two to 4 months after the development of MI without ST segment elevation, followed by the development of regression models to predict functional activity and outcomes of MI after 6 months, three clinical groups were formed: patients 60-74 years old with MI, grade 1-2 hypertension without frailty; patients 60-74 years old with MI, grade 1-2 hypertension and pre-frailty; patients aged 60-74 years with MI, frailty and grade 1-2 hypertension. It was found that the deterioration of functional activity and geriatric status 6 months after MI in patients with frailty and hypertension is due to frailty, cognitive disorders, hypomobility syndrome, sarcopenia and malnutrition. The leading prognostic markers associated with a high risk of MI complications after 6 months in elderly patients with frailty and hypertension are LDL, apoliprotein A1 and apoliprotein B. The created regression models make it possible to predict the functionality of patients after MI in combination with frailty and AH and, in case of its decrease, promptly implement a diversified strategy.
