All over the world there is a clear tendency, which is manifested in an increase in the number of people in older age groups. Gerontological patients have features of the functioning of the immune system, such as a decrease in the activity of the immune response, a tendency to immunodeficiency states, a higher incidence of autoimmune diseases, a decrease in the effectiveness of immunization, and pronounced age-related involution of the thymus. Due to a decrease in immunological reactivity, clinical manifestations and diagnosis of surgical and oncological diseases will have specific features, such as: blurring of the clinical manifestations of the disease, a higher incidence of destructive, complicated forms of the disease, often not typical changes in laboratory parameters, "mutual burden syndrome" , high operational and anesthetic risk, longer rehabilitation, an increase in the number of postoperative complications and unfavorable outcomes.
