PATHOPHYSIOLOGY OF LEFT VENTRICULAR DIASTOLIC DYSFUNCTION IN OLDER AGE PATIENTS WITH CHRONIC HEART FAILURE WITH PRESERVED EJECTION FRACTION

Summary:
Background: The number of patients with chronic heart failure with preserved ejection fraction (CHFpEF) is increasing every year. The pathophysiological aspects of CHFpEF remain controversial, especially in the group of elderly patients. The aim of the study: analysis and generalization of modern data on the pathophysiology of CHFpEF in patients of older age groups. Materials and methods: Russian and English-language sources presented in search engine. Results: Analysis of the scientific literature has shown that pathophysiological patterns of CHFpEF development in patients of older age groups are inextricably linked with diastolic dysfunction of the left ventricular myocardium and are explained by the processes of natural aging and comorbid pathology. This combination triggers a systemic inflammatory response in the coronary microvascular endothelium, leads to a change in myocardial compliance and extensibility. It also confirms the concept of the continuity of systolic-diastolic processes. Conclusion: In patients of older age groups, diastolic dysfunction is one of the key mechanisms explaining the development of CHFpEF. Currently, the problem of the occurrence of CHFpEF and its progression among patients of older age groups requires further study.
