In this article, the authors develop the original biopsychosocial models of activating care for elderly and senior patients at home. According to the expert assessment, the activating care model includes the following main elements: selection care, patient activation measure (including activities that affect biological and psychosocial determinants the quality of life in patient elderly and senior age); evaluation of the effectiveness care. Model of biopsychosocial activating care for geriatric patients in the home, aimed at improving the quality of life, to modify biological and internal environment of patient, mainly maintaining by activate kinesitherapy and cognitive gymnastics, minimizing passive care to achieve the highest possible level of self-reliance functioning, and not just the implementation of care in order to meet basic needs.
