The aim of the work was to study the effectiveness of a comprehensive, systematic approach to the management of patients with chronic obstructive pulmonary disease as a disease with manifest systemic manifestations, developed on the basis of the clinical and epidemiological data and neyroimmunoendokrinologicheskih. The study on the clinical efficacy of the developed scheme of rehabilitation of older patients with COPD, taken from the position of "medicine based on evidence" shows the systemic exposure of advantage, which is focused also on the extra pulmonary manifestations of the disease. It is significantly greater increase in physical performance indicators (according veloergometry test and a 6-minute walk test), quality of life. The studies also found that improved as a result of the rehabilitation of indicators of physical performance and quality of life are preserved even after the termination of preventive and rehabilitative measures. This is revealed us an important advantage of system of rehabilitation measures to the traditional tactics of management of patients for which the aftereffect with this pathology is not typical. It is important to note that physical rehabilitation methods used contribute to reliable positive dynamics of cytokine status without affecting the used indicators of oxidative status, which is an important indicator of their effectiveness.
